How does partial credit work on Next Generation NCLEX questions?

NCLEX items with more than one scoring key can use partial credit. The scoring method depends on the item, so a partly correct response does not always earn points in the same way.

What are the three partial-credit methods?

The methods are plus/minus, zero/one and rationale scoring. Read the NCSBN scoring explanation together with the official item previews.

MethodWhat earns pointsWhat to watch
Plus/minusCorrect selections add points; incorrect selections subtract them. The item floor is zero.Selecting every option can erase points.
Zero/oneCorrect responses earn points; incorrect ones earn none.Follow the stated number of selections.
RationaleLinked responses must be correct together to earn the points for that relationship.A correct cause paired with an incorrect effect can lose the linked credit.

Can you show a simple plus/minus example?

In an illustrative item with three correct options, choosing two correct options and one incorrect option earns one point under plus/minus scoring: 2 − 1 = 1. This is an arithmetic example, not an official NCLEX item.

Do not assume every matrix row, bow-tie part or multiple-response item uses that exact calculation. First read the instruction about how many responses to choose and what relationships the item asks you to show.

Should I guess every possible answer?

Select the options supported by the case, rather than every option that could be true in another situation. A distractor may describe valid nursing care but fail to answer this item.

If a prompt says to select a fixed number, use that number. If it asks for all applicable options, judge each against the actual information. Partial credit rewards useful knowledge; it does not make unsupported selections free.

How should I review a partly correct item?

Review the points you lost and the reason for each lost point. A four-point result on a six-point task contains more information than a single “wrong” label.

Record whether you missed a cue, chose an unsupported action or broke a linked relationship. Then find another task that tests that same reasoning. Familiarize yourself with the official sample formats before exam day.

How can I put this into my next study session?
